Insurance Information

We participate with most insurance plans including the exchange plans, Medicare, Medicare Advantage Plans and Medicaid. Please call our office for more details. Insurance contracts can change frequently, so please call us for more details. When you call to make your appointment, be sure to provide us with your complete insurance information. We will call your insurance company to verify your benefits and coverage a day or two in advance of your appointment.

Please note that routine eye examinations may not be covered by your insurance – it all depends upon your plan. Some managed health plans and HMO plans require an insurance referral from their primary care physician, so keep that in mind when planning your appointment. And finally, please remember to bring your insurance cards with you on the day of your appointment.
