Key-Whitman Eye Center is pleased to offer Neuro-Ophthalmology and Headache Medicine services to patients in the Dallas location.
Deborah Friedman, MD, MPH, Neurologist and Neuro Ophthalmologist with a specialty in Headache Medicine, will be available to see patients who are referred by a physician. Patients referred for neuro-ophthalmology evaluation must have seen an ophthalmologist or optometrist prior to being scheduled. Patients referred for headache evaluation must have previously seen a neurologist.

What to bring to your appointment:
It is critical that you review the New Patient Checklist and complete the New Patient Packet (and Headache Medicine New Patient Questionnaire, if applicable) linked below before your appointment. You will need to have your referring physician send all clinical referral notes including any Visual Field testing and OCT results (if performed) by fax to 214-754-5903. MRI and/or CT scans must be saved on CD and it is your responsibility to have them mailed to our office at 11442 North Central Expressway, Dallas, TX 75243 or bring them with you to your appointment. Please print and complete the forms linked below and bring them to your appointment. If you are unable to download or print your forms, please arrive 30-45 minutes prior to your appointment time to complete them.
Insurance is not accepted:
Although Dr. Friedman is out of network with all insurance including Medicare and Medicaid, you will receive a receipt with the diagnosis codes and procedure codes which you may forward to your insurance company for consideration (not applicable to Medicare or other governmental insurance). The new patient fee for Neuro-Ophthalmology visits is $500.00 and the fee for Headache Medicine new patients is $650.00. We accept cash, checks up to $500.00, MasterCard, Visa, AMEX and Discover as well as offer payment plans up to 24 months deferred interest through CareCredit and Alphaeon.
