The one-minute self-evaluation can be taken at home and will provide you immediate and valuable insight about vision correction procedures that may be a good fit for your vision needs. Once you complete the brief questionnaire, your information will be sent to a Key-Whitman vision specialist for follow up and to make an appointment.
The questionnaire contains a few basic questions to help gauge your vision needs. For the most accurate results, have your current prescription for each eye handy (if applicable).
